FST315 is the mature circulating follistatin isoform. The FST gene encodes a 344-amino-acid precursor; processing yields mature FST315. FST288 arises through alternative splicing, lacks the 27-amino-acid C-terminal sequence present in FST315, and is more heparan/cell-surface associated, while FST315 is more circulating.

Identity and isoform sources support FST315/FST288 biology. NCT01519349 and NCT02354781 concern FST344 gene transfer and must not be used as evidence for direct FST315 protein administration, pharmacokinetics, dosing, efficacy, or safety.
