Quadruple Agonists: What Comes After Triple Agonists?
Quadruple agonists are an emerging area of metabolic research involving compounds designed to activate four biological targets rather than one, two or three.
But “quadruple agonist” does not describe one specific combination of receptors—and four targets does not automatically mean a compound is more effective or safer.
Researchers are testing several different four-target strategies. Most remain preclinical. NA-931 is notable because human clinical research has been reported.

Triple, quadruple and quintuple tell you how many targets are involved—not which targets they are or how well a drug works.
NA-931 — Human Research
Published clinical research describes NA-931 as an orally administered small-molecule quadruple receptor agonist targeting IGF-1, GLP-1, GIP and glucagon receptors.
NCT06615700 was a randomized, double-blind, placebo-controlled dose-escalation study evaluating safety, tolerability, pharmacokinetics and pharmacodynamics.
A 2025 ADA conference abstract reported 74 NA-931-treated participants and dose-dependent mean body-weight reductions reaching 6.4% over 28 days.
Evidence qualification: These findings were reported in a published conference abstract, not a complete clinical-trial manuscript.
Phase 2 Research
A 2025 ADA conference abstract reported preliminary topline findings from NCT06564753, a 13-week randomized, double-blind, placebo-controlled Phase 2 study.
That specific abstract reported mean body-weight reduction reaching 14.8% from baseline and 13.2% relative to placebo at the highest reported dose.
Important evidence note: another published conference report from this research program presented somewhat different numerical results. Until fuller clinical reporting resolves the discrepancy, the Handbook does not treat one percentage as the definitive Phase 2 result.
What About Muscle Loss?
ADA conference reports state that investigators observed no muscle loss.
That finding does not establish muscle preservation as a proven clinical benefit of NA-931. More complete body-composition evidence is needed before making that stronger claim.
Phase 3: Protocol Is Not Results
A 2026 publication describes a Phase 3 protocol evaluating NA-931 alone and with oral semaglutide in 366 participants.
The publication states that topline results will be reported later.
The protocol describes what researchers intend to test. It does not establish that NA-931, the combination, or proposed synergy, durability or body-composition benefits are effective.
NA-931 + Tirzepatide: Registered Research
NCT06732245 is a registered Phase 2 study designed to evaluate NA-931 alone and with tirzepatide in adults with overweight or obesity.
The ClinicalTrials.gov record currently lists the study as not yet recruiting and reports no results.
Quadruple Doesn’t Always Mean the Same Four Targets
GLP-1R / GIPR / GcgR / Y2R
Researchers reported unimolecular peptide tetra-receptor agonists targeting GLP-1R, GIPR, the glucagon receptor (GcgR) and Y2R.
This is preclinical molecular and cell-based research. It is not human weight-loss evidence.
GLP-1 / GIP / Glucagon / Gastrin
A published ADA 2026 conference abstract reported a four-target strategy involving GLP-1, GIP, glucagon and gastrin, including in-vitro experiments and animal experiments in mice.
This is conference-level preclinical evidence. Human efficacy has not been established, and mouse findings do not establish expected human weight loss.

What Comes After Quadruple?
VRX-0075 is an experimental five-target agonist involving GLP-1, GIP, glucagon, amylin and calcitonin receptors.
The evidence discussed here is preclinical. The published ADA 2026 report includes in-vitro work and obese-rat experiments. Its reported comparison with Retatrutide was performed in obese rats, not in humans.
What This Research Does Not Establish
